1. Professional Use Requirement

By signing this agreement, the Renter confirms they are a licensed professional in good standing (e.g., Massage Therapist, Esthetician, Physical Therapist, Chiropractor, Acupuncturist, etc.) and will use the equipment for professional purposes only.

2. Rental Details and Pricing

  • $35 per table, per day

    • 4+ days, $30/day

  • Minimum: 1 table / 1 day

  • Rental period begins on the agreed delivery or pickup date and ends upon return

3. Deposit & Payment

A deposit of 50%  is required to reserve all bookings.
Full payment must be completed 1 day prior to delivery or pickup.

4. Delivery & Pickup

  • Local delivery includes locations within 25 miles

  • Free pick up in North Scottsdale Available

  • Delivery Fee: $50 per table

  • Pick Up Fee: $50 per table

  • Delivery outside of 25 mile radius may incur added handling fees

  • Renter agrees to be present and available at agreed delivery/pickup times

  • Late returns may result in an additional daily rental charge.

5. Condition, Care, & Responsibility

These tables are part of a shared ecosystem of care.

We ask that all renters:

  • Use tables as intended for professional services

  • Keep surfaces clean and protected

  • Return tables in the same condition they were received

  • Notify us of any damage or concerns right away

You are responsible for the equipment during your rental period, including any damage, loss, or theft.

6. Damage, Loss, or Theft

Renter agrees to submit clear photos of all rented equipment at the end of the rental period prior to pickup. Failure to do so may result in forfeiture of deposit.

  • 1 full table photo (open)

  • 1 close-up of surface

  • 1 close-up of legs/frame

Renter agrees to cover the full cost of repair or replacement for any damaged, lost, or stolen equipment.

Replacement cost per table (if needed): $__699__ (you’ll fill this in)

7. Cancellation Policy

  • Cancellations made 48_ hours/days before rental: full refund (you choose—recommend 48 hours)

  • Cancellations made 24_ hours/days before rental: partial refund

  • Late cancellations less than 24 hours may result in loss of deposit

8. Liability Waiver

MasPaz Massage LLC is not responsible for any injury, loss, or damages resulting from the use or misuse of rented equipment.
